Membership is open to Intermediate Care Facilities (ICF/DD-H, ICF/DD-N) and is based on six beds per home. Please complete all membership fields.

Associate membership is open to companies/organizations that support the DSN membership via their service offerings. Membership is subject to approval by the DSN Board. Please disregard the questions on the membership form that are specific to ICF/DDN homes.
